Abstract
Background\r\nPublic Private Partnerships (PPPs) are contractual agreements\r\nbetween government agencies and private sector, aimed at\r\nimproving the efficiency of public investments. PPPs have\r\nincreasingly been adopted in public health to design, build,\r\nfinance, maintain and operate hospitals. This study was aimed\r\nto perform a systematic review and a time trend analysis of\r\npublished PPP hospital experiences around the world.\r\nMethods\r\nA systematic literature search was carried out, from 1990 to\r\n2014, by a specific search algorithm through PubMed and\r\nScopus databases and a snowball search. Resulting studies were\r\nselected by two reviewers independently according to title,\r\nabstract and full text. Inclusion criteria were a PPP applicative\r\nexperience and a hospital setting. The trend of interest in the\r\nscientific community was analyzed by calculating the 5-year\r\naverage annual publication rate. PRISMA guidelines were\r\nadopted for reporting the study results.\r\nResults\r\nOut of the 519 retrieved studies, 158 were finally selected. Of the\r\nlatter, 73(46.2%) described experiences from Europe (37 from\r\nthe UK), 36(22.8%) from Asia (15 from India), 35(22.2%) from\r\nNorth America, 16(10.1%) from Africa, 9(5.7%) from Australia,\r\n6(3.8%) from Latin America. The annual publication average\r\nrate was 10.2(range 0–23). The 5-year average annual publication\r\nrate steadily increased from 0.4 in the years 1990–1994 to\r\n18.8 in 2010–2014 (p< 0.01), with a steeper increase since 2005–\r\n2009 and a peak of 23 in 2014.\r\nConclusions\r\nPPP in hospital setting draws increasing attention from\r\nscientific community. Despite a wider adoption in the UK,\r\nIndia and North America, PPP recently spread around the\r\nworld. The observed time trend could also be related to\r\nconstraints from the worldwide socio-economic crisis. A\r\nfurther PPP diffusion in public health is foreseeable. PPP\r\noutcomes in different economic contexts will highlight its\r\nefficacy in hospital management and its potential role in the\r\nfuture economic framework.\r\nKey messages\r\n The scientific community is showing an increasing interest\r\ntowards PPP adoption and outcomes as a result of PPP\r\nspreading experiences also relatable to socio-economic\r\nconstraints\r\n The adoption of PPP in hospital settings could represent a\r\nuseful strategy enabling governments to meet their objectives\r\nmore effectively by improving the efficiency of public\r\ninvestments.
