Abstract
[Autom. eng. transl.] In recent decades, health systems have been hit by profound changes. To the effects of the demographic transition (ie the transition from a demographic regime characterized by high birth rates and mortality to a situation characterized, on the contrary, by low birth rates and mortality) the consequences of the epidemiological transition, described by Omran (1971 ) as the transition from an epidemiological framework in which acute diseases prevailed to a scenario in which the main causes of morbidity are due to chronic-degenerative diseases. These changes have led to the transition from a care model based on care (a performance aimed at curing an acute pathology) to a model based on care (continuous assistance aimed at caring for a patient suffering from chronic degenerative disease and disability), through a personalized, highly humanized path, oriented towards satisfying complex needs, preferably located close to the patient's life worlds (Ardigò, 1997). This paper intends to investigate the changes that have affected welfare health systems and describe some of the changes underway, through a critical assessment of the challenges that welfare systems must address
