Abstract
[Autom. eng. transl.] A young person practicing volleyball at a competitive level, during a game, suddenly feels a strong chest pain with dyspnea, in the absence of blows or traumas, which worsens until becoming worsening and requiring access to the emergency room. After the first investigations more aimed at excluding cardiac causes, through imaging tests the cause of symptomatology is identified in spontaneous pneumomediastinum, in the absence of associated pneumothorax. The complex physiopathological mechanism underlying the problem is the so-called Hamman-Macklin mechanism, (which is illustrated and demonstrated in full in the slides of oral communication). After the resolution of the clinical picture and the normalization of the radiographic profiles, however, a dyspnea caused by physical effort, difficult to interpret diagnostic, persists even after several months. Several diagnostic hypotheses and tests are carried out; the process is shown in oral communication. NB RX and CT images are shown during the presentation, with explanation of the pathognomonic signs of pneumomediastinum, before and after resolution.
