Abstract

[Autom. eng. transl.] Introduction. Scientific research is increasingly attempting to identify the predictive factors of patient self-care skills which is one of the most important variables for ensuring the efficacy and sustainability of chronic patient care processes (Bodenheimer et al., 2002; BosTouwen et al ., 2015Dimatteo et al., 2002; Fogarty et al., 2002; Aggarwal et al., 2015). In this perspective, there is a growing awareness of the importance of a care relationship that is oriented towards promoting patient autonomy (Norris et al., 2002; Greene et al., 2015). Research has recently shown that patient engagement - defined as the patient's psychological predisposition to be an active protagonist of his / her own health care path - constitutes a key factor in determining self-care behaviors and that a therapeutic relationship oriented to support patient engagement is associated with better clinical and psychosocial outcomes (Bennett, Fuertes, Keitel, & Phillips, 2011; Golin, Dimatteo, & Gelberg, 1996; Lee & Lin, 2010). Based on these premises, the study presented in this paper has set itself the objective of exploring the mediating effect of patient engagement between the doctor's relational orientation and self-care behaviors, the level of health literacy and health. mentality of patients. Method. A cross-sectional study was conducted on a representative sample of patients with chronic disease, adults and Italians. The self-report questionnaire was composed of validated tools to measure the different variables involved in the rationale of the study (Relational orientation of the doctor Healthcare Climate Questionnaire (Gremigni, Sommaruga, & Peltenburg, 2008); Health literacy Brief 3 item health literacy questionnaire (Chew , Bradley, & Boyko, 2004); patient engagement Patient Health Engagement Scale (Graffigna et al., 2015); Mental health status SF-12v2 mental health subscale (Pezzilli et al., 2006); self-care Morisky Greene Levine Scale) (Morisky, Green, & Levine, 1 Descriptive analyzes and structural equation models were conducted with the help of the AMOS software. Results. A total of 1195 questionnaires were collected. The results show that both tested structural equation models show a good fit with data and theoretical hypotheses. The comparison between the fit indices however shows that the model that includes patient engagement as a mediation variable explains with the relationship between the doctor's orientation towards engagement and self-care behaviors, the level of health literacy and the mental health of patients is more effective. Conclusions. the present study has shown that a relational style of the physician supporting patient autonomy is significantly associated with clinically relevant outcomes. However, patient engagement seems to play a fundamental role in mediating the relationship between these variables. This result may have significant implications in the orientation of clinical practice, the training of health professionals and patient education activities.
