Abstract
[Autom. eng. transl.] Diabetes is a chronic disease characterized by the presence of high levels of glucose in the blood (hyperglycemia) and is due to an altered quantity or function of insulin. Diabetes can lead to chronic complications that can be prevented, or whose progression can be slowed down, through strict control of all risk factors. The management of long-term Diabetes Complications (CLTD) can be achieved through territorial assistance that provides the patient with information and means for an adequate metabolic control, in order to avoid the onset of frameworks of decompensated diabetes that would make the hospitalization. Among the Prevention Quality Indicators of the Agency for Healthcare Research and Quality, the rate of potentially avoidable hospital discharges for CLTD makes it possible to assess, indirectly, the appropriateness of the territorial management of diabetes. The analysis obtained through this indicator is to be considered in an integrated manner with what has been described, regarding the hospitalization for diabetes mellitus, in the Chapter "Metabolic diseases" of this Report.
