Abstract

[Autom. eng. transl.] Introduction. The variability in clinical practice represents one of the most current and complex topics in Public Health. This variability is also observed in the management of diabetes, a widespread chronic condition burdened, if not correctly managed, with multiple complications. In Italy, the prevalence of diabetes is 5.5% and its frequency is constantly increasing. Patient care, given the onerous social, economic and health impact of the disease, represents, today, one of the essential elements on which the organization of health systems must be focused. Methods. Through a review of the literature, we have investigated the issue of variability in treatment in Italy, taking into consideration the therapeutic management of diabetes and access to the hospital for the complications associated with it. Results. From the consultation of the data of the National Esiti 2014 Program, it is clear that the rate of hospitalization for short and long term complications of diabetes presents an important inter and intra-regional heterogeneity. Variability also exists in the therapeutic management of the disease which, as emerges from the 2013 OsMed Report, presents significant differences both in terms of consumption and gross expenditure per capita. It is possible to identify, in general, a growing North-South gradient that, if on the one hand it can be traced to the prevalence of the disease, on the other it can mask deficiencies in the control of the disease at the territorial level. Conclusions. The analysis of the data reveals an unjustified variability, in terms of both consumption of resources and health of the clients, which indicates that, to date, there is no uniformity of assistance in our country. The "government of variability" therefore represents a strategic necessity functional to the uniform and equitable improvement throughout the national territory of the control of diabetic disease and requires local and regional accountability.
