Abstract

[Autom. eng. transl.] Introduction In the literature a positive impact of the nurse's leading role in the treatment of chronicity in primary care was reported. The objective of the present work is to compare the effectiveness of the Self Management (SMS) support carried out by the general practitioner compared to that performed by the nurse in relation to health outcomes in chronic patients or patients suffering from multi-chronicity. Methods A systematic literature review was conducted by querying the following databases: Medline, CINAHL, Web of Science and Scopus. English language articles published from 2000 to 2014 were included. The methodological quality assessment was also conducted through the "Cochrane Collaboration's tool for assessing risk of bias". Results 22 out of 7153 studies met the inclusion criteria for a total of 7317 patients. Studies have been stratified by pathology: diabetes (5), cardiovascular disease (11), chronic obstructive pulmonary disease - COPD (3), multi-chronicity (3). Through the various parameters described in the documents and the tools for measuring self-management skills, a greater efficacy of the nursing intervention than that of the GP was highlighted. Respectively in 80% of studies for cardiovascular pathology, in 66% for COPD, in 50% for diabetes. No difference in efficacy between GP and nurse was found in the case of multi-chronicity. Conclusions These results highlight a decisive role of the nurse in supporting the self-management of chronic patients, determining a different distribution of skills among professionals and characterizing the phenomenon of task shifting.
