Abstract

[Autom. eng. transl.] Clinical assistance pathways (PCA) are a fundamental tool in the management of complex hospitals and pathologies that require high integration. They allow a better management of resources, a reduction in waiting times and a global management of the patient. The “A Gemelli Foundation” hospital has developed several PCAs in recent years, including those recently approved for patients with solid brain injury. The PCA dedicated to the patient with solid brain injury was developed under the guidance of the operational unit of the "Clinical Government" with the participation of all the professional figures involved in the care. On the basis of the scientific evidence and of the hospital reality of reference, a flow-chart was created in which the phases of treatment and the places of care are shown in the vertical axis. Three sub-groups focused respectively on defining the epidemiological context of patients accessing our structure due to brain injury; draw up the flow-chart; define a set of quality indicators for monitoring the care provided. The final work was approved by all the professionals involved. The work groups produced a final report including flow charts and quality indicators concerning 5 evaluation areas submitted for approval by all stakeholders involved. The main results were the streamlining of the patient's journey and the introduction of multi-disciplinary meetings (Tumor Board) as a clinical decision tool. With the application of the PCA the management and integration of the hospital with the territory have also improved. The PCA is a valid tool to respond to the need to improve care with attention to the resources used and efficiency. The involvement of all professionals working in the treatment of the disease is a key element.
