Abstract

[Autom. eng. transl.] Introduction. The psychoanalytic diagnosis has allowed us to trace with ever greater clarity the possible links between Depressive Disorders, personality and defensive styles (McWilliams, 1994; PDM Task Force, 2006). Of particular interest is above all Blatt's attempt to identify two mental assets - anaclitic and introjective - within Depressive Disorders (Blatt, 1974, 2004, 2008), as it opens up perspectives for understanding a clinical situation, often challenging from the point of psychotherapeutic view, for which the personological aspect is frequently underestimated. Method. Single-case study. Inside the following tools were used: SCID I (First et al., 1997), SWAP-200 (Shedler, Westen, 1998), DMRS (Perry, 1990), HAM-D (Hamilton, 1960) and DEQ ( Blatte et al., 1976), with a specific focus on the latter. Results. From the scores obtained it emerges how an anaclitic configuration prevails in the patient, which remains rather constant over time, indicating the presence of an interpersonal dependence and a type of depression characterized by feelings of helplessness, loneliness and fragility, related to chronic fears of abandonment and lack of protection. The SWAP-200 has highlighted the presence of scores beyond the average referring to cluster A (schizoid and schizotypal personality disorders) and to cluster C (avoidant personality disorder), which, however, are less elevated at the end of psychotherapy. The defensive profile of the patient, outlined with the DMRS, appears to be characterized by the mechanism of denial, strongly reduced at the end of therapy: this reduction is associated with the remission of the Major Depressive Disorder, present at the beginning of the treatment and detected through the SCID I and the HAM-D. Conclusions. In addition to highlighting the success of psychodynamic treatment (Busch et al., 2004), the contribution confirms the link between clinical variables (Depression), personality and defensive.
