Abstract
[Autom. eng. transl.] Hemodialysis treatment allows patients with renal insufficiency a good level of survival while waiting for a possible organ transplant. However, therapy greatly affects the patient's life. About 80% of chronic patients suffer from severe fatigue, which strongly affects the level of quality of life (QoL). Fatigue is a multidimensional symptom consisting of two dimensions: "physical" and "motivational". The first is associated with obvious physical limitations but the recovery mechanisms retain all their effectiveness, allowing the body to regain its strength by means of an adequate rest period. The second is more complex and refers to a central type of tiredness, often associated with sleep disorders, pain and affective and cognitive alterations. Previous studies in other groups of patients with chronic diseases suggest the use of psychological interventions to reduce fatigue but, such interventions are not available for hemodialysis patients. This study evaluates the presence and level of fatigue hypothesizing a comorbidity with previous experiences of anxiety and depression and how the motivation mechanisms are influenced by fatigue and cause a significant impact on QoL. Evaluation scales were administered to a sample of about 50 dialysis patients over the age of 18 and without neurocognitive disorders. The Fatigue Severity Scale (FSS) was used to measure the impact of fatigue on the patient's motivation and social function. For the investigation of the reward mechanisms we administered the BIS / BAS Scale, which allows us to investigate the mechanisms of motivational disposition, the Behavioral Activation System (BAS) and Behavioral Inhibition System (BIS) which, in behavioral terms, can be translated into motivational tendency to approach or move away from rewards and punishments. Finally, to assess the possible presence of anxiety and depressive disorders, the State-Trait Anxiety Inventory (STAI-Y) and the Back Depression Inventory (BDI-II) were used respectively. The results show a linearity relationship between the sense of fatigue and the levels of depression: a. Fatigue is directly proportional to symptoms such as sadness, frustration, irritability, difficulty concentrating and loss of interest b. The reduction in motivation to action, guided by the BAS reward system, is inversely proportional to depressive symptoms. Therefore it would seem possible to hypothesize their indirect correlation with the sensation of chronic fatigue. This study shows that the feeling of fatigue is not attributable exclusively to physiopathological factors of renal failure and dialysis treatment but to a central type of tiredness, linked to the psychological level. A psychosocial intervention would be useful to improve the quality of life of the hemodialysis by attenuating the "fatigue" symptom.
