Abstract
[Autom. eng. transl.] Territorial Assistance (AT), like any complex system, evolves and shapes itself at the same time
with the socio-demographic, economic and political scenario of reference. The purpose of this Chapter is to provide
a snapshot of TA at a national level which, although incorporated in an aligned definitional and operational framework
to the contemporary context, through recommendations based on scientific evidence, may also result
forerunner of possible, new organizational configurations.
In the current Edition of this Chapter, in particular, a system on the eve is described and characterized
of profound structural and functional changes. Starting from the definition and trying to clarify the term
minology, AT means a Primary Care system aimed at a population residing in a particular
mined geo-political area (1). The two terms are therefore substantially overlapping. Assistance
Primary is configured as a complex system of health offering to individual and community recipients,
characterized by evolutionary relationships between the client, community stakeholders and the various actors involved in the various settings
welfare. Primary Assistance aimed at the first contact (with the treatment of non-complex acuity, the
mulation of a diagnostic suspect and gatekeeping to further specialized levels), but addressed, above all,
to take charge of chronic health conditions, through a life-course approach. Specifically, by “taking
in charge "means a process in which, alongside administrative, organizational and first treatment access,
continuity of assistance is guaranteed according to a relational, information and management dimension to
chronic health conditions that not only concern states of disease, but also include conditions of fragility,
disability and exposure to risk factors. In this regard, it is emphasized the need to insert the taking charge
of the individual in a much broader logic of taking charge of the entire community of reference, in which it governs
nance system, according to a Population Medicine approach, define strategies and interventions calibrated to
to population groups identified on the basis of a stratified and integrated assessment of social and social needs
health. From this, a targeted and anticipatory response can also be achieved towards the needs of the individual, through the
proactive and elective recruitment of pre-classified subjects for which multi-professional and multi-disciplinary teams
plinaries, on the basis of a multidimensional evaluation, formulate, activate, monitor and adapt Plans
Individualized Assistance.
