Abstract

[Autom. eng. transl.] The aging of the population and the increase in the prevalence of chronic pathologies are two of the most described phenomena in the scientific literature of the last decades. Chronic pathologies, to date, represent the main share of disease and use of health services. The data of the National Institute of Statistics for the year 2015 report that 38.3% of residents in Italy declared to be affected by at least one of the main chronic pathologies and 19.8% from multicronicity (at least 2 chronic pathologies). Within the over 75 year old resident population, these percentages reach 85.2% and 65.4% respectively. These demographic and epidemiological changes determine a complexification of the need for social and health care in the population which, in turn, requires the National Health Service (NHS) to remodel the care dynamics. In response to this, it is necessary to switch from "performance" type assistance, based mainly on the hospital setting, to "take charge" logics of the individual, to be implemented at a territorial level through the implementation of a system based on the Primary Assistance. This Chapter presents the results of core indicators that aim to measure the assistance processes aimed at specific targets identified by the nature of the need or by the different ages of life; the identified targets include: - assisted with conditions of need linked to non-self-sufficiency; - assisted in children. Monitoring of assistance processes through the aforementioned indicators allows continuous improvements to be made, with the ultimate aim of addressing the specific configurations of the need for assistance for the particular targets identified in the most appropriate way.
