[Autom. eng. transl.] INTRODUCTION: The use of alcohol in the workplace has been the subject of numerous evaluations by the world of prevention and there has been a variegated and sometimes different regulatory orientation at regional and local level. The legislation in force in our country entrusts employers with the task of combating the use of alcohol by workers who carry out tasks at risk of accidents at work or for the safety, security or health of third parties. The LaRA group (Risky Workers for Others) has set itself the task of assessing whether the provisions fully guarantee the protection of the health and safety of workers and third parties, without prejudice to the civil rights of workers. MATERIALS AND METHODS: A consensus document was produced in the comparison between occupational physicians, hygienists, forensic doctors, toxicologists, psychiatrists, general practitioners, jurists, bioethicists, and social partners. The document was compiled evaluating in comparison the numerous existing rules and the jurisprudential opinions on the subject, through network and formal meetings. RESULTS: The importance of a correct approach to the problem of the use of alcohol was highlighted, particularly in particularly work environments risky for workers and third parties. However, the current legislation has gaps and inconsistencies that make it difficult to implement uniform prevention practices. Faced with a general sharing of basic preventive concepts, there are differences in the provisions of local and regional authorities that do not allow for the application of a unified strategy on the national territory. Unlike what has been established by the regulations so far, the employer's obligation to monitor compliance with the prohibition of alcohol intake at the workplace should not be carried out "only" through a competent doctor, whose institutional role should be trusted and non fiscal . Furthermore, some categories for which the use of alcohol is envisaged are not currently subject to health surveillance. Workers driving road vehicles not exposed to other risks can be subjected to health surveillance only in regions where regional laws so provide. In other cases, the risk assessment for third parties and health surveillance are considered by some lawyers acceptable, by others illegitimate. And not infrequently the doctor in charge of alcohol checks and alcohol dependencies faces an ethical dilemma, since the communication of the results to the employer or the licensing bodies may not comply with the obligation of professional secrecy. The emphasis placed only on controls has also led companies and supervisory bodies to neglect educational and rehabilitative aspects in general. The involvement of general practitioners, educators and specialist services is essential to address the problems of alcohol abuse in an information / training, recovery and rehabilitation perspective. The few studies available indicate that the rules are still poorly applied and non-compliance is not always subject to controls, favoring the diffusion of very dissimilar behaviors. CONCLUSIONS: All employers should assess the risk for third parties deriving from the use of alcoholic substances a shared strategy (policy) on alcohol. The control of this risk in the workplace requires a clearer definition of the roles of the Control and Supervisory Bodies and of the Competent Doctor and the transition from a reactive attitude to a proactive one of all the parties involved. The contribution of General Practitioners appears to be indispensable in a general framework of training and information and health education for the adoption of law
